This session will discuss a process improvement project performed at the Massachusetts General Hospital Interventional Radiology Department that focused on preprocedure anticoagulation medication management. The importance of care coordination in Interventional Radiology setting, the specific steps taken to prepare and roll out this project among a multidisciplinary team, how we empowered staff to participate and discuss how we are sustaining this project will be reviewed.
